2. Appointments and Cancellation Policy

To provide quality care to all patients, we require a minimum of 24 hours' notice for any appointment cancellations or rescheduling. Failure to provide such notice may result in a cancellation fee.

3. Payment Obligations

Patients are responsible for full payment of all services rendered. While we may assist with insurance claims, the ultimate financial responsibility lies with the patient. Payments are due at the time of service unless otherwise agreed.

You agree to receive administrative and marketing communications via the contact methods provided to us. You may opt out of marketing communications at any time.

5. Limitation of Liability

Smile American LLC shall not be liable for any indirect, incidental, or consequential damages resulting from the use of our services or website, except as prohibited by Florida law.
